VACANCY NOTICE – CASE MANAGER
The Human Resource Management Department on behalf of the Ministry of Youth Empowerment, Ageing and Disabilities wishes to invite suitably qualified persons to fill the position of a Case Manager.
The chosen candidate will have the responsibility to assess, plan, implement, coordinate, monitor, and evaluate the options and services necessary to address the individual and familial needs (health, social care, educational, employment) of individuals and clients approaching the Ministry. Their goal is to enhance client functioning by delivering and facilitating connections to services that fulfill their unique individual and familial requirements.
Duties and Responsibilities:
- To review all intakes completed and referrals made on behalf of individuals seeking assistance from the Ministry.
- To conduct a needs assessment of the individual and where applicable their familial unit
- To create a care plan collaboratively with the individual which would identify the activities to be undertaken to address the needs are expressed or identified.
- To refer the individual to services and programs that can address the needs as outlined in the care plan.
- To monitor the individual’s actions taken to access the services and opportunities to address the needs as outlined in the care plan.
- To prepare regular case notations on the outreach, advocacy and actions taken to accomplish the care plan activities.
- Prepare monthly/quarterly reports to the Permanent Secretary
- To maintain an up-to-date listing/registry of network of service providers (public sector agencies, private sector agencies, charitable individuals, charities, CBO’s, FBO’s, NGO’s)
- To conduct exit or graduation interviews complete with satisfaction surveys with all clients who are no longer in need of services referred to or provided by the Ministry.
- To conduct regular visits and assessments with service providers to determine issues, challenges, and amendments to processes and procedures involved in case management.
- To conduct process evaluations and reviews of the procedures and processes used in case management.
- Liaise with local vendors and service providers to assist with the procurement of supplies and equipment for the Ministry.
- Attending meetings, workshops and providing feedback to inform planning and decision-making of the Ministry.
- Assist with the Emergency Management responsibilities of the Ministry.
- Demonstrate flexibility to work outside of normal working hours including in the evenings, on holidays, vacation and/or at weekends and emergency situations.
- Ability to maintain regular, punctual attendance, consistent with regulations that govern civil servants.
Minimum Qualifications and Experience:
- A bachelor’s degree in Social Work, Social Sciences or Human Services or any other relevant field is desirable.
- At least one year’s experience in a related position (case worker, case manager etc).

In addition to the requirements above, the post holder must have the following skills:
- Must be able to work under pressure and meet deadlines, display soft skills, conflict resolution skills, maintain a positive attitude and provide exemplary customer service.
- Must be able to organize work and achieve outcomes and meet deadlines.
- Ability to work independently, take initiative in the best interest of the Ministry and carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- Ability to perform the essential job functions including meeting qualitative and/or quantitative productivity standards safely and successfully.
- Proven experience in establishing collaborative partnerships with local organizations, government, and communities.
- Strong facilitation skills with the ability to listen, empathize and interact well with diverse stakeholder groups.
- Proven high level verbal and written communication skills with the ability to present, prepare professional reports, policies, strategies, grants, and correspondence.
- Must be able to talk, listen and speak clearly in person and on the telephone.
- Project management experience and interpersonal skills
- The ability to manage several priorities at any one time.
WORKING CONDITIONS:
- Normal working environment
- Will be required to sign and affirm the Oaths of Secrecy and Confidentiality
- Will be required to attend activities and programs on weekends and evening hours.
- Will be required to maintain regular, punctual attendance, consistent with regulations that govern civil servants.
- Will be required to travel locally to meet with stakeholders to gather and share information related to the care plans of clients.
- May be required to work overtime to meet deadlines and successfully complete client and Ministerial activities.
Salary: (K33-K38) $64,092-$78,768 per annum
